In the ancient Light Gun game Gumshoe for the NES, there is a fatal flaw in the code during the final level. While I am not sure what triggers it to happen, (and, thankfully, it doesn't always occur) when it does, it is unrecoverable. The screen will go blank and neither the gun or the controller will do anything about it. (I think the sound still works, but that is it.)
It's one of the very few bugs that I know of in a Nintendo title that made it all the way to the production line.
